Your Opinion Matters: Shape Your Local Health Care

The Champlain Local Health Integration Network (LHIN) needs 10 minutes of your time to help shape your local health care. We are setting strategic priorities for the region’s health system for the next three years, through the LHIN’s Integrated Health Service Plan 2016-19, and your input will help us prioritize and plan to make better use of our resources.

A little more about the LHIN: it is a local Crown Agency that plans, coordinates and funds health services in the following health sectors:
  • Hospitals,
  • Community Care Access Centre (home care)
  • Addictions and Mental Health Agencies
  • Community Support Services (such as Meals on Wheels)
  • Community Health Centres
  • Long-Term Care Homes. 

The LHIN’s mandate is to ensure the services are well-organized, appropriately funded, and meet the needs of residents of all ages so that people receive the care they need and deserve in a timely way.

For the next three years, the LHIN is proposing an Integrated Health Service Plan (IHSP) that:
  • Aligns and advances provincial priorities (Patients First: Action Plan for Health Care)
  • Builds on local health accomplishments
  • Takes into account what we’ve learned about the health and health care experiences of people in the region who use services, and
  • Meets local needs.

To ensure quality and sustainable health care as well as achieve the health outcomes our population deserves, we must make better use of our many existing resources. Among the challenges ahead of us are changing demographics and resource constraints. We will all need to work together to improve the system and achieve the best possible results.

Please provide your feedback through our anonymous survey “Developing the Integrated Health Service Plan 2016 – 19” . It will take about 10 minutes to complete, and closes on Aug 27, 2015.

Your input will help us finalize the IHSP 2016-19 by late November (it will launch April 1, 2016).
